Registration Form for Programs and Activities Involving Minors

Please complete and submit this form to register any program or activity that will include minors as participants.This may include athletic camps, academic camps, licensed childcare facilities, instructional programs, internships with individual staff or faculty, and other organized programs and activities intended for minors. Program Information:

1.         All programs or activities, including internships, where minors will be working in labs must complete the “Guidelines & Checklist for Hosting Minors in Labs” found on the Office of Science Outreach’s website at http://bit.ly/osolabs.

2.         All programs or activities, including internships, where minors will be working in science labs must review and adhere to Environmental Health & Safety’s (EH&S) Health and Safety Requirements for Minors in Laboratories at Stanford University found at http://bit.ly/minorslabs.

3.         Program Sponsors (i.e. responsible PI, staff or faculty member) must ensure minors complete required online and lab specific safety training. Contact EH&S at (650) 723-0448 for guidance on appropriate trainings and/or consult the Training Needs Assessment tool in STARS.

4.        Consultations or approvals from EH&S and/or medical surveillance/clearance may also be required depending on the type of work proposed. Contact EH&S Laboratory Safety Program at (650) 723-0448 for guidance.

By submitting this form, I certify full understanding of Stanford University’s Protection of Minors policy and agree to comply with the requirements of that policy to ensure all minors participating in University sponsored programs will remain safe during their participation.  Further, the Program Director and Sponsor Contact/Representative understand that compliance with the policy must be satisfied and recorded by Stanford Conferences before the program may take place, despite any services and/or facilities bookings that may have been secured and/or any financial obligations that may have been incurred.
